About The Position

We are currently seeking a Full Time - Population Health Quality Coordinator. This position will be based out of our new Palmdale clinic. This position is responsible for leading the quality improvement activities needed to perform favorably on contracts and ACO financial incentives as well as to deliver overall high-quality care as a Medical Assistant to appropriate patient populations. This position will work closely with primary care providers and clinical staff to achieve key measures of success. Will be responsible for closing gaps in care and accurately coding patients’ severity of illness in their documentation. This position participates in collaborative meetings and process improvements involving ACOs, insurance companies’ quality program managers and the primary care practices. Effectively communicates, providing constructive feedback to providers and staff alike on meeting quality goals. Works collaboratively with payers to understand their quality data and communicate to the Providers and staff. Responsible for performing a wide variety of patient care duties which include assisting in examination and treatment of patients under the direction of a physician.

Requirements

  • High School Diploma or equivalent required.
  • Minimum of 3 years’ experience as a Medical Assistant.
  • Fluent with medical terminology.
  • Current BLS/CPR certification required.
  • Demonstrates compliance with Healthcare policies and procedures at all times.
  • Ability to set priorities and appropriately organize workload and complete assignments in a timely manner.
  • Demonstrates ability to relate to clinical personnel and medical staff, as well as ability to interact well with the public.
  • Must have knowledge of PC and applications.
  • Participates in decision-making and process improvement.
  • Ability to Understand Managed Care Quality Incentives.
  • Proficiency in basic computer programs and operating systems, such as Microsoft Office.
  • Excellent communication, organizational and interpersonal skills.

Nice To Haves

  • Technical School/1 Year College preferred.
  • Bilingual in English and Spanish preferred.
