About The Position

In the role of Population Health Manager you'll oversee care coordination staff, strengthen community partnerships, and use data-driven insights to enhance care delivery, streamline operations, and elevate quality outcomes. A strong candidate has demonstrated experience in nurse quality assurance (QA), care management, utilization management, or clinical quality improvement, and a solid understanding of population health principles.

Requirements

  • Bachelor’s degree in Health Administration, Nursing Public Health, Human Services, or a related field is required.
  • Excellent communication, leadership, and change-management skills are essential, along with the ability to manage multiple initiatives, align stakeholders, and drive sustainable improvements in population health outcomes.
  • Experience in quality reporting, data-driven decision-making, and program management within healthcare or public health environments is strongly desired.

Responsibilities

  • Oversee day-to-day operations of the Care Coordination Team, ensuring efficient service delivery and alignment with population health goals
  • Provide administrative oversight and operational support to nursing, LNA, and MA staff involved in care coordination and population health activities
  • Develop, implement, and refine workflows to support effective referral management, care transitions, and follow-up
  • Ensure care coordination services are delivered in a timely, accessible, and person-centered manner
  • Establish and monitor key performance indicators (KPIs) related to access, timeliness, quality outcomes, and care coordination effectiveness
  • Use data and quality metrics to identify trends, gaps, and opportunities for improvement
  • Lead continuous quality improvement initiatives focused on patient outcomes and system efficiency
  • Facilitate strong internal collaboration across clinical, behavioral health, and support service teams
  • Support effective internal and external care coordination, including transitions between levels of care
  • Partner with community providers, hospitals, social service agencies, and payers to strengthen referral networks and service continuity
  • Build and maintain relationships with community partners to support coordinated, whole-person care
  • Provide leadership, guidance, and operational support to care coordination and medical support staff
