About The Position

Oak Street Health®, a part of CVS Health, helps older adults stay healthier and live fuller lives through its 230 centers across 27 states. Their value-based care model aims to consistently deliver better patient experiences and outcomes. As a Population Health Manager, you will play a critical role in supporting Oak Street Health patients, providers, and the business by coordinating and implementing key Population Health priorities. This role involves working on programs and interventions targeted to improve quality and cost of care, acting as an effective cross-team leader and support for providers, care teams, operations team members, and Treehouse team members. Content areas will include transitions-in-care, pharmacy and medication adherence, HEDIS/Stars, accurate clinical documentation, and medical cost management, with a focus on avoidable admissions. This requires building relationships with clinicians and center team members. This is a fast-paced, high visibility role focused on driving innovation and improvement within an Oak Street division, aligned with a geographic division to achieve results. The role reports directly to the Regional Vice President, with a dotted line to the central Population Health team.

Requirements

Minimum of 3 years of applicable experience in healthcare / population health / working with healthcare providers; or other similar fields
Strong communication skills
Ability to build the trust and confidence of a wide range of stakeholders
Leadership potential and ability to provide guidance to help team members execute on population health priorities
Demonstrated history of creative problem-solving
Strong organization and detail orientation skills
Strong skills in Excel and Google Apps
Bachelor’s degree required
US work authorization

Responsibilities

Lead strategic planning, execution and the ongoing refinement of Population Health programs in the respective region.
Work side by side with regional leaders and center care teams to support the Oak Street care model including but not limited to clinical documentation excellence, workflow optimization, and avoidable admission prevention.
Assist with creating solutions to support accurate clinical documentation and avoidable admission prevention strategies that are scalable across multiple centers.
Work closely with functional leaders in the central office to assist with strategy and program design for accurate clinical documentation and medical cost / admission efforts.
Lead the analytics, insights, and project management that drive population health execution in the region.
Be accountable for medical scribe cohort recruitment, retention, performance and engagement in the respective region to ensure clinical documentation excellence.
Accountability to key population health metrics including quality measures, utilization measures, and financial measures.
Other duties, as assigned.
