About The Position

Provide direct primary medical care to all participants of the program. The physician performs comprehensive and interval histories and physicals, evaluates and treats episodic illnesses, and refers participants to medical specialists when indicated. The role includes managing hospital care as the attending physician, coordinating with the care team and consultants, and supporting discharge planning. The physician also manages participants’ care in nursing homes, participates in family conferences, and takes part in assigned team meetings, committees, and work groups.

Requirements

M.D. or D.O. degree from an accredited school; work experience may not substitute for this education requirement.
At least 2–3 years of experience, including a minimum of one year working with a frail or elderly population.
Current license to practice medicine in South Carolina.
BLS certification.
Proven clinical leadership.
Ability to work well with others in a team environment.

Responsibilities

Perform comprehensive histories and physicals for new referrals.
Perform interval histories and physicals for established participants quarterly or as indicated.
Evaluate and treat participants with episodic illnesses.
Refer participants to medical specialists as indicated.
Admit participants to the hospital and provide attending physician responsibilities for their management.
Provide regular updates to the care team during hospital admissions.
Use specialty consultants judiciously.
Coordinate discharge planning with the team.
Manage participant care in nursing homes through regular visits and telephone contacts with nursing home staff as required.
Admit nursing home participants to the hospital when necessary.
Participate actively in family conferences.
Participate in team meetings, committees, and work groups as assigned.
