About The Position

CenterPlace Health is seeking a Physician to join their team at the North Port Health Center. This role provides comprehensive general medical care within a primary care delivery system, including consultative services. The physician will be part of an on-call team ensuring 24-hour coverage and must maintain current medical knowledge and high ethical standards. A key expectation is to provide support, mentoring, and oversight to Advanced Practice Providers and Novice Physicians.

Requirements

  • Graduation from an approved school of medicine or osteopathy.
  • Three (3) years of approved residency in their specialty field.
  • Current and continuous maintenance of all required CME credits.
  • A minimum of 2 years clinical experience or equivalent working in a medical practice.
  • Current license to practice medicine in the State of Florida.
  • Able to analyze information and evaluate results to choose the best solution and solve problems.
  • Dedicated to ensuring the highest quality of care to patients.
  • Excellent communication skills.
  • Friendly and professional demeanor.
  • Technical skills including intermediate typing skills.
  • Knowledge of proper, safe, efficient usage of current equipment/software.
  • Knowledge and ability to use Office 365 programs including Outlook and Word.
  • Possession and maintenance of Board Certification in area of specialty (Internal Medicine/Family Practice).

Responsibilities

  • Provide medical care within a primary care delivery system.
  • Provide comprehensive general medical care, including consultative services.
  • Participate as part of the on-call team for 24-hour coverage.
  • Maintain current medical, diagnostic, and therapeutic acumen.
  • Support, mentor, and provide oversight to Advanced Practice Providers and Novice Physicians.
  • Elicit history and conduct physicals.
  • Provide proper medical care as allowed by medical training and licensure.
  • Provide preventive medical care per standard of care.
  • Provide consultative services for peers.
  • Order appropriate labs, EKGs, x-rays and recognize abnormal findings.
  • Prescribe and dispense medications and treatments following appropriate standard of care guidelines.
  • Initiate requests for consultation or referral with appropriate professionals, clinics, or other health care resources.
  • Assess and triage patients.
  • Educate patients on all aspects of health self-maintenance.
  • Provide short-term, individual, or family teaching as appropriate.
  • Assist with development and implementation of patient educational materials.
  • Accurately and efficiently enter documentation into the EHR.
  • Ensure all EHR documentation is completed accurately and timely.
  • Cosign charts as required timely.
  • Ensure all charts are closed out in Athena timely.
  • Demonstrate basic understanding of EHR order entry and workflow, workflow dashboard, document and task management, problem flowsheets, troubleshooting documents in the clinical inbox.
  • Actively participate in quality improvement projects.
  • Participate in ongoing peer review of medical records, active participation in committee assignments, and QA/QI activities.
  • Participate in and present Continuing Medical Education activities.
  • Comply with Center policy and procedures and promotes and complies with NCQA PCMH standards of care.
  • Assist in identification, evaluation, and intervention of quality issues in the clinical setting.
  • Participate in program required committees.
  • Maintain ethical and professional standards.
  • Supervise nursing and clinical staff activities.
  • Provide supervision, education, and direct assistance when appropriate to Novice Physicians, Advanced Practice Providers, and student providers.
  • Perform other related duties as indicated or when requested by the Chief Medical Officer or their designee.
  • Assist with management of patient flow to assure effective and efficient care.
  • Cover for other providers within the scope of licensure as needed.
  • Apply patient evaluation and management processes to clinical situations.
  • Maintain use of universal precautions.
  • Complete daily/day-end activities as assigned.
  • Complete all required training and attend meetings as assigned.
  • Protect/observe patient confidentiality per policies and procedures.
  • Recognize and respond appropriately to violent/abusive situations, bomb threats, fire and emergency situations.
  • Complete a background check.
