About The Position
We are seeking an adaptable Post Acute Primary Care Physician (MD/DO) to oversee post-acute clinical services at partner Skilled Nursing Facilities in Melbourne and Merritt Island, Florida. The physician will provide hands-on care through daily rounds, serve as the primary on-site clinical leader, manage higher-risk patients, and supervise a dedicated team of Nurse Practitioners and Physician Assistants. The schedule is Monday through Friday, 8:00 AM to 5:00 PM, with coverage split between the two facility locations.
Requirements
- MD or DO degree.
- Board certified or board eligible in Family Medicine or Internal Medicine; board certification is not required.
- Active Florida medical license; ACN (Area of Critical Need) license holders are accepted and welcome to apply.
- Ability to commute between facilities in Melbourne and Merritt Island.
Responsibilities
- Conduct daily rounds, splitting coverage between facilities in Melbourne and Merritt Island.
- Perform initial assessments and evaluations for newly admitted patients.
- Manage acute updates and coordinate care plans for higher-risk, fragile patients.
- Monitor, supervise, and collaborate closely with regional Advanced Practice Providers (APPs).
