About The Position
We are seeking a dedicated and compassionate Post Acute Primary Care Physician (MD/DO) to join the post-acute care team in Jacksonville, Florida. The physician will perform daily rounds at designated Skilled Nursing Facilities (SNFs), providing transitional and long-term care for residents. The role includes managing complex clinical cases, admitting new patients, and providing clinical oversight to a collaborative team of Advanced Practice Providers (APPs). This is a full-time, on-site position, five days per week, Monday through Friday, from 8:00 AM to 5:00 PM.
Requirements
- MD or DO degree.
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatrics; board certification is not required.
- Active Florida medical license in good standing. ACN (Area of Critical Need) license holders are accepted.
- Strong communication skills.
- Passion for geriatrics or post-acute medicine.
Responsibilities
- Conduct daily clinical rounds across assigned Skilled Nursing Facilities in the Jacksonville area.
- Evaluate and examine newly admitted patients and establish comprehensive care plans.
- Directly manage higher-risk, medically complex patients requiring intensive monitoring.
- Provide clinical supervision and mentorship to Advanced Practice Providers and sign off on their charts.
