About The Position
A premier post-acute medical group is seeking an autonomous Post-Acute Care Primary Care Physician (MD/DO) to lead facility-based care in Lake Wales, Florida. The role involves daily rounds across Skilled Nursing Facilities (SNFs), management of high-acuity patients, care of new admissions, and providing structure and supervision to local Advanced Practice Providers (APPs). The position is full-time, five days per week, Monday through Friday, from 8:00 AM to 5:00 PM. The physician will work on-site at designated SNFs in the Lake Wales area.
Requirements
- MD or DO.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine; board certification is not required.
- Active Florida Medical License.
- ACN (Area of Critical Need) license holders are accepted and welcome to apply.
- Strong clinical decision-making skills in a post-acute setting.
Responsibilities
- Perform daily rounds at designated Skilled Nursing Facilities (SNFs) in the Lake Wales area.
- Review, examine, and admit incoming new patients to sub-acute units.
- Directly intervene and manage care for higher-risk or clinically declining patients.
- Supervise and guide local Advanced Practice Providers (APPs), and sign off on their charts.
