About The Position

Open Door Community Health Centers is seeking a Primary Care Physician to join their integrated care team at Fortuna CHC in Fortuna, California. This outpatient practice offers an excellent work-life balance with flexible scheduling, no hospital call, and opportunities for teaching, mentorship, and leadership. The physician will provide comprehensive, patient-centered outpatient care for a panel of patients across the lifespan in a rural Federally Qualified Health Center (FQHC) setting. They will diagnose, treat, and manage acute and chronic medical conditions, emphasize preventive care, and coordinate specialty services. The role is critical in improving access to high-quality healthcare for an underserved rural community.

Requirements

• Successful completion of accredited medical school (MD Degree Required)
• Completion of accredited Internal or Family Medicine Residency Program required
• Current, unrestricted California physician and surgeon license
• Current DEA registration
• Current Board Certification in specialty of practice (or board eligible if a newer graduate)
• Current CPR/BLS certification
• Valid California driver's license with an insurable driving record

Responsibilities

• Provide comprehensive, patient-centered outpatient care for a panel of patients across the lifespan in a rural Federally Qualified Health Center (FQHC) setting.
• Diagnose, treat, and manage acute and chronic medical conditions.
• Emphasize preventive care.
• Coordinate specialty services when needed.
• Work as part of an interdisciplinary care team to improve access to high-quality healthcare for an underserved rural community.
