About The Position

FOX Rehabilitation is hiring licensed Physical Therapist Assistants to provide hands-on care to older adults in Bonita Springs, Florida, and surrounding communities. Care is delivered in patients’ homes and senior living communities through FOX’s Geriatric House Calls™ model. Working with a supervising Physical Therapist, the PTA carries out individualized treatment plans and guides patients through therapeutic exercises and functional activities to help improve strength, mobility, confidence, and independence. The role offers one-on-one patient care, a partnership with a supervising PT, mentorship, and opportunities to grow clinical skills, caseload, and career. PTAs work as part of an interdisciplinary team that includes Physical Therapists, Occupational Therapists, Speech-Language Pathologists, and Fitness Specialists. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. Benefits are available for eligible roles; part-time benefit eligibility depends on meeting a minimum-hours requirement. Compensation is competitive and based on experience and employment type. Requirements

Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain one.
Graduate of an accredited Physical Therapist Assistant program.
Current CPR certification.
Basic computer skills and proficiency in Microsoft Office.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, and extensive walking.
Ability to drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
Ability to wear PPE when required.

Responsibilities

Deliver individualized treatment plans established by the supervising Physical Therapist, adjusting activities based on patient response and progress.
Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities, observing and tracking progress.
Document visits accurately and on time in FOX’s EMR system, following FOX’s Clinical Documentation standards.
Stay connected with patients, POAs, and community referral sources, and help coordinate coverage when needed.
Provide in-home and community-based care, including visits to patient homes and senior living communities.
Work collaboratively with the interdisciplinary care team and supervising Physical Therapist.
