About The Position

FOX Rehabilitation is hiring a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities in and around Bonita Springs, Florida. Using FOX’s Geriatric House Calls™ model, the clinician helps patients improve mobility, independence, wellness, and quality of life. Care may also include telehealth and other virtual services when appropriate. The role offers autonomy to create and manage a treatment schedule, develop lasting patient relationships, and work within a collaborative, interdisciplinary team. FOX offers full-time, part-time, and PRN/Fox Flex employment options. New graduates may be eligible for the Emerging Professionals Mentor Program. Qualified clinicians may also be eligible for H1B sponsorship. Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly.
Ability to provide moderate to maximum assistance with patient transfers and bed mobility.
New employees must be cleared through the Florida Background Check Clearinghouse.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
Document care in FOX’s EMR system in accordance with regulatory standards.
Collaborate with physical therapists, occupational therapists, speech-language pathologists, and fitness specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
Meet physical demands of in-home and community-based care, including CPR, patient transfers and bed mobility, repositioning, standing and walking for extended periods, climbing stairs, and therapeutic activities.
Wear PPE when required.
