Physical Therapy Assistant - Bonita Springs
About The Position
FOX Rehabilitation is hiring licensed Physical Therapy Assistants to provide hands-on, individualized care to older adults in Bonita Springs, Florida, and surrounding communities. Care is delivered in patients’ homes and senior living communities through FOX’s Geriatric House Calls™ model. The PTA works closely with a supervising Physical Therapist to carry out individualized treatment plans, guide patients through therapeutic exercises and functional activities, and support strength, mobility, confidence, and independence. The role offers one-on-one patient care in a team-based, interdisciplinary setting alongside Physical Therapists, Occupational Therapists, Speech-Language Pathologists, and Fitness Specialists. FOX provides mentorship from the supervising PT and opportunities to develop clinical skills, caseload, and career. Available employment pathways include full-time, part-time, and PRN/FOX Flex roles; benefits and eligibility depend on the role and applicable minimum-hours requirements.
Requirements
- Hold a valid Physical Therapist Assistant license in the state or states of practice, or be eligible to obtain one.
- Graduate from an accredited Physical Therapist Assistant program.
- Have current CPR certification.
- Have basic computer skills and proficiency in Microsoft Office.
- Meet the physical demands of in-home and community-based care, including the listed lifting, CPR, patient-transfer, mobility, and movement requirements.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
Responsibilities
- Deliver individualized treatment plans established by the supervising Physical Therapist, adjusting activities based on patient response and progress.
- Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities, observing and tracking their progress.
- Document visits accurately and on time in FOX’s EMR system in accordance with FOX’s Clinical Documentation standards.
- Stay connected with patients, their POAs, and community referral sources, and help coordinate coverage when needed.
