About The Position

Amazing Care Home Health Services provides in-home care and partners with clinicians, caregivers, and operational teams to deliver timely, personalized support. The Physical Therapy Assistant supports pediatric patients in the Lower Valley coverage area by helping implement individualized treatment plans, assessing patient abilities and limitations, educating patients and families, and documenting care and progress. The role works with children with varying diagnoses and challenges and collaborates on reports, discharge planning, and team meetings. The schedule distinguishes part-time as 24 visits or fewer and full-time as 25 visits or more. Full-time employees are eligible for a $1,000 sign-on bonus, subject to restrictions and paid after 90 days of employment.

Requirements

• Licensed Physical Therapy Assistant in Texas.
• Strong communication and interpersonal skills to engage with children and families.
• Proficiency in electronic medical records (EMR) documentation.
• Ability to develop and implement personalized treatment plans for diverse patient needs.
• Flexibility and adaptability to work with children with varying diagnoses and challenges.

Nice To Haves

• Experience in pediatric home health or a related field preferred.

Responsibilities

• Assist in developing and implementing individualized treatment plans tailored to each child's needs.
• Support assessment of patients' abilities and limitations using standardized and non-standardized tests.
• Educate patients and families about home exercise programs, adaptive equipment, and daily living strategies.
• Stay current with best practices, regulations, and professional standards of care.
• Maintain detailed documentation of treatment sessions, patient progress, and changes in condition.
• Prepare reports for patient evaluations, discharge planning, and team meetings.
