About The Position

Emblem Home Health & Hospice is hiring a Physical Therapy Assistant (PTA) to provide services in Mesa, Chandler, and Gilbert. This is a full-time position with a Monday–Friday schedule. The PTA provides hands-on care under the supervision of a Physical Therapist, helping patients work toward their goals while benefiting from a flexible schedule. The organization emphasizes team support, mentorship, ongoing education, and a collaborative culture.

Requirements

• Licensed Physical Therapy Assistant (PTA) in the State of Arizona.
• Graduate of a CAPTE-approved PTA program and passed the national exam.
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Deliver physical therapy treatments according to the Physical Therapist’s and physician’s plan of care.
• Guide patients through therapeutic exercises and mobility training.
• Monitor and report patient progress and changes in condition.
• Educate patients on mobility aids and prosthetic devices.
• Maintain accurate clinical notes and participate in team meetings.
