About The Position

Choice Health at Home is hiring a Physical Therapist to assess patients, develop and deliver individualized physical treatment, and support quality, continuous home health care. The therapist evaluates the effects of treatment and adjusts care to maximize patient benefit, follows infection-control and universal precautions, communicates effectively with patients, families, team members, and other health care professionals, and promotes agency philosophy and administrative policies. Documentation must be completed within company guidelines.

The position involves variable work hours set with the supervisor, manager, and applicable operating-license documentation. The company is open Monday through Friday, 8:00 a.m. to 5:00 p.m. CST. Eligible employees may receive mileage reimbursement for use of a personal vehicle, and company vehicle eligibility may be available after 90 days, subject to program requirements and availability. A $5,000 sign-on bonus is advertised.

Requirements

• Successful completion of an accredited physical therapy program and state licensure examination.
• Valid Physical Therapy license.
• CPR certification.
• Valid driver's license.
• Valid auto insurance.
• Able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
• Able to lift, carry, pull, or push up to 100 pounds.

Nice To Haves

• One year of home health experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform and document an initial examination, evaluate data, identify problems, and determine a diagnosis before intervention.
• Plan, prepare, or carry out individualized physical treatment programs to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction.
• Evaluate treatment effects at various stages and adjust treatments to achieve maximum benefit.
• Apply infection-control and universal precautions while providing patient care.
• Ensure continuity of quality patient care with appropriate documentation.
• Communicate effectively with patients, families, team members, and other health care professionals.
• Promote agency philosophy and administrative policies.
• Complete and maintain documentation within company guidelines.
