About The Position

The Lutheran Care Center at Poughkeepsie is seeking a Physical Therapist to join its team at a nonprofit nursing and rehabilitation facility. The role manages the professional components of Physical Therapy evaluations and treatments, assesses residents, establishes goals and plans, and monitors treatment progress in accordance with New York State requirements. New graduates are welcome. The position pays $52.00 per hour and offers flexible scheduling. A $5,000 sign-on bonus is offered for full-time Physical Therapists; the bonus is paid in installments and eligibility and repayment terms apply. Employees must be fully vaccinated for COVID-19, MMR, and PPD.

Requirements

Completion of a Physical Therapy program.
Valid New York State Physical Therapy license.
Fully vaccinated for COVID-19, MMR, and PPD.

Responsibilities

Manage the professional components of Physical Therapy evaluations and treatments.
Assess and evaluate residents and establish goals and plans.
Re-evaluate goals and treatment plans in accordance with New York State code.
Document required medical record progress notes per New York State code.
Make recommendations to the Director and/or Physician regarding a resident’s response to treatment.
Provide input as a member of the Comprehensive Care Planning Team.
Oversee Physical Therapy Assistants and Physical Therapy Aides.
Attend mandatory in-services and provide Physical Therapy in-services as needed.
