About The Position
Concept Rehab is seeking a Physical Therapist (PT) – PRN to join our therapy team in Waterford, MI. This is a great opportunity for a PT looking for flexible scheduling, supplemental hours, or the ability to work around other professional and personal commitments while making a meaningful difference in the lives of patients. Requirements
- Current Michigan Physical Therapist license
- Graduate of an accredited Physical Therapy program
- Strong clinical and communication skills
- Patient-centered approach
- Ability to work independently and as part of a team
Nice To Haves
- Skilled nursing or post-acute rehabilitation experience is a plus
- New graduates are welcome to apply
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide skilled physical therapy interventions to improve strength, mobility, balance, coordination and functional independence
- Monitor patient progress and modify treatment plans as needed
- Collaborate with PTAs and other members of the interdisciplinary team
- Educate patients, families and caregivers
- Participate in care planning and discharge planning
- Complete accurate and timely clinical documentation
- Help patients achieve their highest level of function and independence
