About The Position
Concept Rehab is seeking a PRN Physical Therapist (PT) to join our team in Bay City, Michigan. This is a great opportunity for a PT looking for flexible scheduling while providing meaningful, patient-centered care in a supportive rehabilitation environment.
Requirements
- Current Physical Therapist (PT) license in the state of Michigan
- Graduate of an accredited Physical Therapy program
- Current or eligible Michigan PT licensure
- Strong clinical, communication and organizational skills
- Compassionate, patient-focused approach
Nice To Haves
- Skilled nursing, rehabilitation or post-acute experience is a plus, but not required
Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans
- Provide skilled physical therapy interventions to improve mobility, strength, balance and function
- Monitor patient progress and adjust treatment plans as needed
- Document evaluations, treatments and progress accurately and timely
- Collaborate with PTAs, nursing staff, physicians and other members of the healthcare team
- Help patients achieve greater independence and quality of life
