About The Position
FOX Rehabilitation is hiring a licensed Physical Therapist to provide one-on-one care to older adults in North Wilmington, Delaware, and surrounding communities. Care is delivered in patients’ homes and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians may also incorporate telehealth and other virtual care into their schedules. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Physical Therapists evaluate patients, develop individualized plans of care, and provide evidence-based treatment that supports mobility, wellness, longevity, and functional independence. The position is part of a clinician-operated, interdisciplinary practice focused on meaningful relationships, clinician-driven decisions, and helping older adults remain independent. FOX lists full-time, part-time, and PRN / FOX Flex employment pathways. Full-time and part-time roles are described as salaried, with benefits for full-time and benefits eligibility for part-time based on a minimum-hours requirement. Compensation varies by experience and employment type. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program. H1B sponsorship is available for qualified clinicians.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Wear PPE when required.
