About The Position

Interim HealthCare is hiring a Home Health Physical Therapist to provide personalized, home-based therapy to patients in South and/or North Miami-Dade County. The role focuses on helping patients regain mobility, strength, and functionality after illness or injury. Therapists work with patients in their home environments, assess progress, and adapt care to support each patient’s plan and goals. The position offers flexible assignments, autonomy, and work-life balance, with a 1:1 therapist-to-patient ratio. The therapist may collaborate with a home health team that includes nursing and other therapy or care professionals.

Requirements

• Graduate of an accredited Physical Therapist program.
• Active Physical Therapist license in Florida.
• At least 1 year of physical therapy experience; home healthcare experience is preferred.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Good clinical judgment, strong interpersonal skills, dexterity, and compassion.

Responsibilities

• Provide home-based physical therapy to patients who have lost mobility, strength, or functionality due to illness or injury.
• Work with a home health team, which may include an RN, LPN, CNA, HHA, OT, and SLP, to support the patient’s plan of care and goals.
• Observe patients’ movements, conduct assessments, document progress, and modify care plans as needed.
• Assist patients with exercises and hands-on therapy to manage pain and increase mobility.
• Educate patients about their plan of care, goals, exercises, and self-care.
• Coach family members on the plan of care, therapy goals, and safely assisting with exercises.
