About The Position

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence in their homes. You will deliver personalized care focused on functional improvement to help patients overcome limitations and improve their quality of life. This per-diem role involves providing physical therapy in patients’ homes and coordinating care with physicians, the patient team, and family members.

Requirements

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid state driver’s license.
• Maintain personal vehicle liability insurance meeting applicable state minimum limits or the stated coverage amounts, whichever is higher.
• Complete TB screening if selected for the role.

• One year of experience as a Physical Therapist.

Responsibilities

• Plan and administer prescribed physical therapy treatment to patients in their homes to help them attain the highest possible level of physical function.
• Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement rehabilitation programs consistent with the physician’s Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Promptly and thoroughly document patient observations, interventions, and evaluations.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family.
• Submit required evaluations, plans, progress notes, and discharge summaries.
