Home Health Physical Therapist-Per Diem
About The Position
CenterWell Home Health is hiring a per-diem Home Health Physical Therapist who reports to the Clinical Manager. The therapist provides personalized physical therapy to patients in their homes, helping them regain strength, mobility, independence, and physical function. Treatment is based on the physician’s Plan of Treatment and patient goals. The position includes coordinating care with physicians, therapy assistants, home health aides, and other members of the patient’s care team. This is a patient-facing role subject to Humana’s tuberculosis screening program. A valid state driver’s license and personal vehicle liability insurance meeting applicable requirements are required. For candidates applying from a Florida work location, the position is subject to Florida Level 2 background screening.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid state driver’s license.
- Maintain personal vehicle liability insurance at the state-required minimum limits or the stated alternative limits, whichever is higher.

Responsibilities
- Plan and administer prescribed physical therapy treatments in patients’ homes to help them attain the highest possible level of physical function.
- Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement rehabilitation programs consistent with the physician’s Plan of Treatment and the patient team’s goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Promptly and thoroughly document patient observations, interventions, and evaluations.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family.
- Submit required evaluations, plans, progress notes, and discharge summaries.
