About The Position

CenterWell Home Health is seeking a per-diem Home Health Physical Therapist who reports to the Clinical Manager. The therapist provides personalized physical therapy to patients in their homes, helping them regain strength, mobility, independence, and functional ability. The role includes assessing patients, planning and administering treatment in line with the physician’s plan, coordinating with the care team, and documenting and reporting patient progress. The position is patient-facing and requires TB screening, a valid state driver’s license, and personal vehicle liability insurance that meets applicable requirements.

Requirements

Current and unrestricted Physical Therapy license.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Valid state driver’s license.
Maintain personal vehicle liability insurance meeting the applicable state minimum or the stated coverage amounts, whichever is higher.
Complete TB screening if selected for the role.

Responsibilities

Plan and administer prescribed physical therapy treatments in patients’ homes to help them attain the highest possible level of physical function.
Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
Develop and implement rehabilitation programs consistent with the physician’s Plan of Treatment and the patient team’s goals.
Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
Promptly and thoroughly document patient observations, interventions, and evaluations.
Report patient progress to the physician, Clinical Manager, care team, patient, and family.
Submit required evaluations, plans, progress notes, and discharge summaries.
