Home Health Physical Therapist-Per Diem
About The Position
CenterWell Home Health is hiring a per-diem Home Health Physical Therapist who reports to the Clinical Manager. The therapist provides personalized physical therapy in patients’ homes to help them regain strength, mobility, independence, and quality of life. The role involves assessing patients, planning and delivering treatment consistent with physician orders and patient goals, coordinating care with the healthcare team, supervising support staff, and maintaining thorough documentation. The position is patient-facing and requires participation in Humana’s TB screening program. It also requires a valid state driver’s license and personal vehicle liability insurance meeting applicable minimum limits. Candidates applying from a Florida work location are subject to Florida Level 2 background screening.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid state driver’s license.
- Maintain personal vehicle liability insurance at the residing state’s minimum required limits or the stated coverage limits, whichever is higher.
- Complete TB screening if selected.

Responsibilities
- Plan and administer prescribed physical therapy treatment in patients’ homes to help them attain the highest possible level of physical function.
- Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement rehabilitation programs consistent with the physician’s Plan of Treatment and the patient team’s goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Promptly and thoroughly document patient observations, interventions, and evaluations.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family.
- Submit required evaluations, plans, progress notes, and discharge summaries.
