Physical Therapist (PT) - Home Health, Full-Time or Per Diem
About The Position
There For You Home Care Services, a Division of UniversaCare, is hiring Physical Therapists to provide home health visits in the Waterbury, Connecticut area. Full-time and per diem schedules are available. Per diem clinicians accept visits on a per-visit basis according to their availability, while full-time therapists carry a steady caseload and receive hourly pay. The agency is Medicare-certified and describes its team as supportive, with manageable caseloads and clear documentation standards. The work is performed in patients’ homes, based from the Waterbury office at 534 Frost Road. The service area includes Greater Waterbury, the Naugatuck Valley, and surrounding towns in New Haven and Litchfield Counties. Hamden-area visits are also available.
Requirements
- Current Connecticut Physical Therapist license.
- DPT or equivalent degree from an accredited program.
- Valid driver’s license, reliable vehicle, and auto insurance.
- CPR certification.
Nice To Haves
- Home health experience and familiarity with OASIS are preferred; clinicians new to home health are welcome if they have strong clinical skills.
Responsibilities
- Complete initial and ongoing physical therapy evaluations in patients’ homes, including OASIS data collection when PT is the assessing discipline.
- Develop and update individualized plans of care with measurable functional goals.
- Provide skilled interventions, including gait and balance training, strengthening, transfer training, post-surgical and orthopedic rehabilitation, and fall-risk reduction.
- Recommend durable medical equipment and home modifications, and train patients in their use.
- Educate patients, families, and caregivers about home exercise programs and safety.
- Coordinate with nursing, occupational therapy, speech-language pathology, aides, and physicians, and communicate changes in patient status.
- Complete timely visit notes, progress notes, recertifications, and discharge summaries that meet Medicare and agency standards.
