About The Position
Concordia Visiting Nurses is seeking a skilled and compassionate Physical Therapist to provide patient-centered care as part of a supportive home health team. Requirements
- Graduate of an accredited Physical Therapy program.
- Valid state license.
- Passion for working with patients and commitment to high-quality, patient-centered care.
- Exceptional communication and interpersonal skills, including the ability to connect with diverse patient populations.
- Team-oriented approach that fosters collaboration and embraces professional development.
- Desire to make a difference in patients' lives and inspire and motivate them during recovery.
Responsibilities
- Conduct comprehensive evaluations and develop individualized treatment plans to help patients reach their maximum potential.
- Implement evidence-based therapeutic interventions, including manual therapy, therapeutic exercises, and modalities.
- Monitor patient progress, adjust treatment plans as needed, and complete timely, thorough documentation.
- Educate patients and their families about treatment options, rehabilitation techniques, and preventative measures.
- Collaborate with physicians and other healthcare professionals to provide holistic patient care.
- Help create a welcoming and supportive environment where patients feel comfortable and motivated.
