About The Position

Concordia Visiting Nurses is seeking a skilled and compassionate Physical Therapist to provide home health therapy in the Baden service area. The therapist conducts evaluations, develops individualized treatment plans, delivers evidence-based interventions, monitors patient progress, and documents care. The role involves educating patients and families, collaborating with physicians and other healthcare professionals, and supporting patients in a welcoming and motivating care environment. The position offers schedule autonomy, patient-centered work, a collaborative team, paid training, scholarships, and opportunities to specialize. Requirements

Graduate of an accredited Physical Therapy program.
Valid state license.
Passion for working with patients and commitment to high-quality, patient-centered care.

Responsibilities

Conduct comprehensive patient evaluations and develop individualized treatment plans.
Provide evidence-based therapeutic interventions, including manual therapy, therapeutic exercises, and modalities.
Monitor patient progress, adjust treatment plans as needed, and complete timely, thorough documentation.
Educate patients and families about treatment options, rehabilitation techniques, and preventative measures.
Collaborate with physicians and other healthcare professionals to support holistic patient care.
Help create a welcoming and supportive environment where patients feel comfortable and motivated.
