Requirements
- Licensed Physical Therapist in the State of Arizona.
- Degree from a CAPTE-accredited program.
- Passed a state-approved examination.
- Current CPR certification.
- Valid driver’s license, insurance, and reliable transportation.
Responsibilities
- Develop and update individualized care plans.
- Provide physical therapy services according to the physician’s plan of care.
- Assess patients’ functional status, mobility, and home environment.
- Educate patients and families on therapy techniques and equipment.
- Maintain accurate documentation and communicate patient progress to physicians.
