About The Position

Stern at Home Therapy is hiring Physical Therapists to provide one-on-one care to a primarily geriatric patient population in patients’ homes throughout Rockland County, New York. The role offers PRN, part-time, and full-time opportunities, with compensation options including salary, W-2 visit-based pay, and 1099 contractor visit-based pay. Clinicians manage their availability and caseload and provide individualized care without double-booking or productivity quotas.

Full-time salary is listed at $92,000–$110,000 annually. Full-time 1099 clinicians using visit-based pay brackets may earn up to $145,000 annually based on visit volume. Per-visit rates vary by employment structure and visit type. Visits are at least 53 minutes, approximately one hour.

Requirements

• Active Physical Therapist license in New York.
• Reliable transportation.
• Strong clinical, communication, and time-management skills.
• Ability to work independently in a home-based setting.

• Home health experience is preferred but not required.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient’s functional needs and goals.
• Manage individualized treatment plans with clinical autonomy.
• Help patients improve mobility, strength, balance, safety, and overall function.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, and members of the clinical team.
• Coordinate with the administrative team regarding scheduling, patient care, and logistics.
• Manage caseload based on availability and employment structure.
