Occupational Therapist (Home Care, 1:1) – New Rochelle, NY – Full-Time
About The Position
Stern At Home Therapy is hiring Occupational Therapists to provide one-on-one care to patients in their homes throughout New Rochelle, New York. The role primarily serves a geriatric patient population and offers full clinical autonomy, flexible caseload management, and administrative support for scheduling, coordination, and patient logistics. Therapists provide 1:1 care at every visit, without double-booking or productivity quotas. Employment structures include PRN, part-time, and full-time options, including W-2 and 1099 arrangements. Full-time 1099 therapists may earn up to $145,000 annually based on visit volume; full-time salary positions offer $92,000–$110,000 annually. Most therapists begin seeing patients within two weeks of joining.
Requirements
- Active New York Occupational Therapist license.
- Reliable transportation.
- Ability to work independently in a home-based setting.
- Strong clinical, communication, and time-management skills.

- Home health experience is preferred but not required.
Responsibilities
- Evaluate and treat a primarily geriatric patient population in patients' homes throughout New Rochelle.
- Develop and manage individualized plans of care with full clinical autonomy.
- Help patients improve independence, safety, mobility, upper extremity function, and activities of daily living performance.
- Provide one-on-one care at every visit without double-booking.
- Complete timely clinical documentation.
- Manage a caseload based on availability and chosen employment structure.
