About The Position

Hartford HealthCare Rehabilitation Network is the premier rehabilitation provider in Connecticut, offering services across a continuum of care including acute care hospitals, long-term care facilities, sub-acute rehabilitation, homecare, outpatient centers, and community settings. With over 40 outpatient locations, they provide physical, occupational, and speech therapy in various specialties. The Outpatient Rehab Clinic in Westport, CT, is a flagship location connected to the Ayer Neuroscience Spine Wellness center, functioning as part of a multidisciplinary team dedicated to improving patient health. This team includes professionals in rehabilitation, pain management, physical medicine, integrative medicine, fitness, nutrition, and behavioral health. The clinic features private treatment spaces and state-of-the-art gyms, supporting specialty programs such as Balance & Falls, Cancer Rehabilitation, Complex Spine, Concussion, Dry Needling, Gender-Affirming Rehab, Headache Management, Chronic Pain Management, Pelvic Health, Sports Medicine, Thoracic Outlet, and Vestibular Therapy. The Physical Therapist (PT) independently evaluates patients, determines treatment strategies, and delivers services to address physical dysfunction or pain, aiming to maximize functional independence. This involves assessing strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, gait, and activities of daily living.

Requirements

  • Graduate of a Physical Therapy Program from an accredited American Physical Therapy Association school, with a BS, master's, or doctorate degree.
  • CT active or temporary license.
  • Will consider new graduates.

Responsibilities

  • Independently evaluates patients, determines treatment strategies, and delivers or delegates services to identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain.
  • Maximizes patient's functional independence through the therapeutic application of physical and chemical agents, exercise, and other procedures.
  • Evaluates strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, gait, and functional abilities in activities of daily living.
  • Collaborates with professionals throughout the facility who provide rehabilitation, pain management, physical medicine, integrative medicine, fitness, nutrition, and behavioral health services.
