Position Overview:
We are seeking a full time Physical Therapist in our West Haven clinic.
Our Outpatient Rehab Clinic in West Haven, CT is connected to our Go Health Urgent Care and PACT Medical Group. This site focuses primarily on sports medicine and orthopedics. It is part of a multidisciplinary team of medical, rehabilitation, and wellness professionals dedicated to improving the health of all patients. This therapist will collaborate with professionals throughout the facility to support our coordinated care model. All of our clinics have ample private treatment spaces and state-of-the-art gyms to support our patient’s recovery.
Specialty Programs in this clinic include Balance & Falls (Safe Steps), Complex Spine, Concussion, Dry Needling, Headache Management, Chronic Pain Management, Sports Medicine, Thoracic Outlet and Vestibular Therapy.

Requirements

• Graduate of Physical Therapy Program from an accredited American Physical Therapy Association school, with a BS, master's, or doctorate degree.
• CT active or temporary license.
• CPR certification.
• Will consider new graduates.

Responsibilities

• Upon referral, the Physical Therapist (PT) performs thorough examination and evaluation of patient’s level of function, including strength, movement, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function by assessing and monitoring vitals, endurance, gait and functional abilities in activities of daily living. The PT recognizes physiological, psychological, and age-related changes in the patient.
• The Physical Therapist (PT) develops an individualized patient-centered treatment plan of care to promote optimum movement function and safety.
• The Physical Therapist (PT) implements, delivers and delegates patient care by utilizing appropriate interventions and clinical decision making including but not limited to modalities such as heat, cold, light, water, sound, electricity, mobilization and therapeutic exercise.
• The Physical Therapist (PT) helps to coordinate patient care with other care providers such as Rehab Therapists, Nurses, Social Workers and participates in patient transition planning and/or patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care.
