Full-Time Home Health Physical Therapist
About The Position
BAYADA Home Health Care is seeking an experienced Physical Therapist for a full-time home health visits position serving adult and geriatric patients in East Providence, Barrington, Warren, Bristol, and surrounding towns in Rhode Island. The Physical Therapist is an integral member of a multidisciplinary health care team that provides rehabilitative care and skilled nursing, helping clients receive the medical care they need to remain at home. The role includes visiting clients in assigned geographic territories, assessing mobility and function, developing and revising individualized care plans, and educating clients and their representatives on rehabilitative care and activities that support health, safety, and independent living. Documentation of observations, interventions, and evaluations is completed using a touch pad tablet.
Requirements
- Current Rhode Island Physical Therapist License.
- Experienced Physical Therapist.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements, including assessments of joint mobility and range, transfer status, stability, gait patterns and appearance, muscle strength and endurance, balance, and safety.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance interventions, and other treatments.
- Continually assess and revise the Physical Therapy care plan, and participate with nursing in the multidisciplinary care plan as appropriate.
- Educate and instruct clients, family members, or other client representatives in rehabilitative care and activities that promote health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services using a touch pad tablet.
