Full-Time Home Health Physical Therapist
About The Position
BAYADA Home Health Care is seeking an experienced, full-time Physical Therapist to provide home health visits for adult and geriatric patients in East Providence, Barrington, Warren, Bristol, and surrounding Rhode Island communities. The Physical Therapist is part of a multidisciplinary health care team providing rehabilitation and skilled nursing services so clients can receive care at home. The role includes conducting home visits and physical therapy assessments, developing and updating individualized care plans, delivering rehabilitative interventions, educating clients and their representatives, and documenting care using a touch pad tablet. A $5,000 sign-on bonus is offered.
Requirements
- Current Rhode Island Physical Therapist License.
- Experience as a Physical Therapist; the posting describes the opportunity as seeking an experienced therapist.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements, including assessing joint mobility and range, transfer status, stability, gait patterns and appearance, muscle strength and endurance, balance, and safety.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance activities, and other interventions.
- Continually assess and revise the physical therapy care plan, and participate with nursing in the multidisciplinary care plan as appropriate.
- Educate and instruct clients, family members, or other client representatives in rehabilitative care and activities that support health, safety, and independent living.
- Accurately document observations, interventions, and evaluations related to client care management and services using a touch pad tablet.
