As the provider of home health therapy services for Accent Care Home Health, we are part of a strong, multidisciplinary team that brings together physical and occupational therapists, physicians, and other healthcare professionals to deliver coordinated, patient-centered care. Requirements
- Graduate (current or pending) from an accredited physical therapy program
- Active or pending state physical therapist license
- Comfort working independently in the home setting
Responsibilities
- Work with a diverse and engaging caseload.
- Provide home health therapy services.
- Collaborate with a multidisciplinary team including physical and occupational therapists, physicians, and other healthcare professionals.
- Deliver coordinated, patient-centered care.
- Participate in mentorship, advanced training and certifications, virtual educational series, and OCS prep collaborative.
- Manage caseloads with thoughtful scheduling and defined territories.
- Minimize drive time between visits.
- Engage in clinical leadership understanding day-to-day challenges.
- Receive individualized support for interests and professional growth.
- Collaborate with AccentCare Home Health colleagues for a comprehensive, team-based approach to care.
- Participate in a Graduated independent caseload ramp-up with ride-alongs.
- Receive documentation training to encourage confidence and efficiency.
- Participate in Advanced Certifications/Training Courses.
- Utilize annual CEU allowance.
- Participate in OCS Prep Collaborative.
- Attend Virtual Educational Series.
- Receive payment for professional, association, and one section dues.
