About The Position
FOX Rehabilitation is seeking licensed Physical Therapists in Goshen, NY, to provide one-on-one care to older adults in private residences and senior living communities. Using the Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. The position offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also incorporate telehealth and other virtual care into their schedules. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and abolishing ageism. The organization offers full-time, part-time, and PRN/Fox Flex employment pathways. Eligible roles include benefits, and qualified clinicians may be eligible for H1B sponsorship. New graduates may participate in FOX’s Emerging Professionals Mentor Program.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy to support patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
