About The Position

Boost Home Healthcare - Omaha provides personalized, coordinated care in patients’ homes. The Physical Therapist helps patients restore function, improve mobility, reduce pain, prevent disability, and achieve greater physical independence. The role includes evaluating patients, developing and updating individualized plans of care, delivering skilled physical therapy, and working with patients, families, and the interdisciplinary home care team. The position offers flexible scheduling, field support, training, development, and negotiable hourly or per-visit pay. Mileage reimbursement is provided.

Requirements

• Graduate of an accredited Physical Therapy program.
• Current Nebraska Physical Therapy license in good standing.
• Current or eligible NPTE certification.
• At least one year of experience in an acute care, rehabilitation, or outpatient setting.
• Strong clinical judgment and ability to work independently.
• Reliable transportation to visit patients in their homes.

Nice To Haves

• At least two years of home health experience is preferred.

Responsibilities

• Help patients restore function, improve mobility, reduce pain, and prevent disability.
• Support patients in achieving the highest possible level of physical independence.
• Participate in interdisciplinary care planning with the home care team.
• Perform comprehensive initial evaluations and OASIS Start of Cares (SOCs), and develop individualized plans of care.
• Administer prescribed physical therapy treatments and skilled interventions.
• Conduct 30-day reassessments, adjust care plans, and update documentation as needed.
• Instruct patients and families on safe mobility, strengthening, and home programs.
• Supervise up to two Physical Therapist Assistants (PTAs) in accordance with Nebraska state guidelines and agency policies.
