Physical Therapist (PT) - Home Health
About The Position
Pivot Home Health is hiring Physical Therapists across Colorado to provide home-based and community therapy for clients across the lifespan. The role may serve adults, pediatric clients, or both, helping clients make progress in the environments that matter most: their homes and communities. For pediatric clients, therapy focuses on fine motor skills, sensory regulation, self-care, and play-based learning to support confidence and participation. For adult clients, therapy supports recovery and independence through adaptive strategies, strengthening, coordination, and functional training. The position offers autonomy in choosing a caseload and setting a schedule, with no minimum visit requirement. Full-time is defined as 20 visits per week. The role includes an iPad for electronic documentation, clinical leadership, 24/7 on-call support, mentorship, training, and professional development. Compensation is $80–$90 per regular visit.
Requirements
- Active Colorado Physical Therapist (PT) license.
- Current CPR certification.
- Strong communication, organization, and collaboration skills.
- Passion for helping individuals reach their full potential through home-based care.

Responsibilities
- Provide physical therapy to clients in home-based and community settings.
- Support pediatric clients with fine motor skills, sensory regulation, self-care, and play-based learning.
- Support adult clients’ recovery and independence through adaptive strategies, strengthening, coordination, and functional training.
- Help clients improve confidence, participation, independence, and quality of life in their homes and communities.
- Complete electronic documentation using a provided iPad.
