About The Position

The Home Health Physical Therapist provides personalized, one-on-one physical therapy to patients in their homes throughout Roanoke, Virginia, and surrounding areas. The role serves patients recovering from illness, injury, or mobility challenges, with individualized treatment plans intended to improve strength, mobility, functionality, patient outcomes, and quality of life. The therapist works independently in a home setting and collaborates with an interdisciplinary care team. The position is part-time/PRN with flexible scheduling. Pay is $45–$100 per visit depending on visit type, plus mileage reimbursement.

Requirements

Graduate of an accredited Physical Therapist program.
Active physical therapist license in Virginia.
Minimum of 2 years of physical therapy experience; home health experience preferred.
CPR certification required.
Knowledge of state and federal home health regulations.
Strong clinical judgment, interpersonal skills, and compassion.
Ability to work independently in a home setting.
Willingness to travel throughout the service area.

Responsibilities

Provide home-based physical therapy to patients recovering from illness or injury.
Conduct patient assessments and develop individualized treatment plans.
Monitor patient progress and modify care plans as needed.
Assist patients with exercises and hands-on therapy to improve mobility and manage pain.
Educate patients on therapy goals, exercises, and self-care techniques.
Coach family members on safely assisting with exercises and patient care.
Collaborate with interdisciplinary team members, including RN, LPN, CNA, HHA, OT, and SLP.
Maintain accurate and timely documentation of patient progress.
