About The Position
Pleasant Hill Post Acute is seeking a compassionate and skilled Physical Therapist to join its in-house rehabilitation team in Pleasant Hill, California. This PRN opportunity offers pay starting at $64–$65+ per hour, scheduling flexibility, and the opportunity to work with an interdisciplinary team focused on resident outcomes. The role provides skilled therapy to help residents improve mobility, strength, and independence. Requirements
- Current California Physical Therapist (PT) license.
- Strong clinical and communication skills.
- Commitment to resident-centered care.
- Ability to work effectively within a multidisciplinary team.
Responsibilities
- Evaluate residents and develop individualized treatment plans.
- Provide skilled physical therapy services to improve mobility, strength, and independence.
- Supervise PTAs and collaborate with the interdisciplinary care team.
- Participate in care planning and discharge preparation.
- Educate residents, families, and caregivers.
- Maintain accurate and timely clinical documentation.
