H2 Health is hiring a full-time Physical Therapist for its outpatient rehabilitation team in Chilhowie, Virginia, with possible coverage at clinics in Damascus and Saltville. The position is open to experienced physical therapists and new graduates. Requirements

Doctor of Physical Therapy (DPT) or equivalent degree from an accredited physical therapy program.
Current Virginia Physical Therapist license or eligibility for licensure.

Commitment to evidence-based care and a collaborative team environment.

Nice To Haves

Outpatient orthopedic experience is preferred but not required.

Responsibilities

Evaluate and treat patients with orthopedic, musculoskeletal, post-surgical, balance, and mobility-related conditions.
Develop and implement individualized, evidence-based treatment plans to improve movement, function, and quality of life.
Provide hands-on interventions, therapeutic exercises, functional training, and patient education.
Monitor patient progress and adjust treatment plans to support meaningful outcomes.
Collaborate with Physical Therapist Assistants and fellow clinicians to provide coordinated, patient-centered care.
Maintain accurate and timely clinical documentation using the EMR and AI-assisted documentation tools.
Provide clinical coverage at the Damascus and Saltville locations as needed.
