About The Position

H2 Health is hiring a full-time Physical Therapist for its outpatient clinic in Horse Cave, Kentucky. The role offers the potential to advance into a Clinic Director position and is open to experienced physical therapists and new graduates. The position is part of a collaborative, clinician-led team focused on high-quality patient care and professional growth.

The Physical Therapist evaluates patients, develops individualized evidence-based treatment plans, provides hands-on therapy, educates patients, monitors rehabilitation progress, collaborates with the clinical team, and maintains accurate and timely clinical documentation.

Requirements

• Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program.
• Active Kentucky Physical Therapist license or eligibility for state licensure.
• Strong clinical reasoning, communication, and interpersonal skills.
• Passion for delivering high-quality, evidence-based patient care.

Nice To Haves

• Outpatient orthopedic experience is a plus, but not required.

Responsibilities

• Evaluate patients and develop individualized, evidence-based treatment plans.
• Provide hands-on physical therapy to improve mobility, strength, and function.
