About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Hilton Head, South Carolina, and surrounding communities. Treatment takes place in private residences and senior living settings, including independent living, assisted living, and memory care communities, using FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, and quality of life through evaluations, individualized plans of care, and evidence-based treatment. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and collaborate with an interdisciplinary team. FOX describes itself as a clinician-operated practice focused on meaningful relationships, evidence-based care, and abolishing ageism. Full-time, part-time, and PRN / FOX Flex pathways are available; compensation and benefits depend on employment type and eligibility. H1B sponsorship is available for qualified clinicians, and new graduates may be eligible for the Emerging Professionals Mentor Program.

Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, extended standing and walking, climbing stairs, and performing therapeutic activities.
Ability to wear PPE when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
