About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in Chester, Virginia, and surrounding communities. Using the Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may also include telehealth and other forms of virtual care.

The role offers autonomy to create and manage a treatment schedule, build lasting relationships with patients, and work in a collaborative, interdisciplinary setting. FOX offers full-time, part-time, and PRN / FOX Flex pathways. New graduates may be eligible for the Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians.

FOX Rehabilitation is a clinician-operated practice focused on meaningful relationships, evidence-based care, and abolishing ageism. Requirements

• Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including occasional lifting of 25–50 pounds, frequent lifting of 10–25 pounds, and constant lifting of up to 10 pounds.
• Ability to perform CPR and provide moderate to maximum assistance with patient transfers and bed mobility.
• Ability to reposition patients, stand and walk for extended periods, climb stairs, and perform therapeutic activities.
• Ability to wear PPE when required.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
• Document care in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
