About The Position
FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one care to older adults in Freehold, New Jersey, and surrounding communities. Care is delivered in patients’ homes and senior living communities using FOX’s Geriatric House Calls™ model, with telehealth or other virtual care incorporated when appropriate. The role focuses on improving patients’ mobility, independence, wellness, longevity, and quality of life. Clinicians can create and manage their treatment schedules and build lasting patient relationships. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and a mission to abolish ageism. The position is part of a collaborative interdisciplinary environment and offers opportunities for professional development and career advancement. Multiple employment pathways are available, including full-time, part-time, and PRN/FOX Flex; this posting identifies the role as per diem. Compensation depends on experience and employment type. The stated New Jersey full-time base pay range is $70,000–$102,000 annually, with part-time and PRN compensation pro-rated.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, repositioning patients, extended standing and walking, stair climbing, and therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and provide evidence-based treatment.
- Deliver therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with physical therapists, occupational therapists, speech-language pathologists, and fitness specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Potentially supervise PTA/OTAs, teach as a Clinical Instructor, mentor emerging professionals, or contribute written or social media content for additional compensation.
