About The Position

Elara Caring is seeking a Physical Therapist to provide compassionate home health care. The role involves delivering physical therapy in patients’ homes and contributing to the company’s mission of providing the right care, at the right time, in the right place.

Requirements

• Graduate of an American Physical Therapy Association (APTA)-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
