About The Position

Adoration Health is seeking a dedicated Home Health Physical Therapist to provide patient-centered care in Salisbury, Maryland, covering the Salisbury area. The role provides one-to-one care in patients’ homes and offers flexible scheduling, independent work with team support, and opportunities for advancement. The therapist evaluates and treats patients according to physician orders, develops treatment plans to support independence and function, coordinates with the care team and families, documents services, and supervises Physical Therapist Assistants and Physical Therapy Aides. The position is full-time and offers a $10,000 sign-on bonus. The listed salary range is $65.00–$70.00 per hour.

Requirements

• Graduate of a Physical Therapist education program approved by CAPTE.
• Current Physical Therapist licensure in the state and American Heart Association CPR certification; CPR certification may be offered and completed upon hire.
• Two years of experience in adult physical therapy.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills; physical therapy skills per competency checklist.

• Home health experience preferred.

Responsibilities

• Perform physical therapy evaluations and treatment based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and/or function.
• Maintain communication among the treatment team, physician, patient, and family members, as applicable.
• Maintain thorough and timely documentation of services to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
