About The Position
GrandCare Health Services is hiring a Physical Therapist to provide in-home physical therapy to patients across Long Beach, Cypress, Lakewood, Cerritos, and surrounding areas. The therapist follows physicians’ plans of care and helps patients regain strength, mobility, confidence, and independence after surgery and other medical conditions. The role offers autonomy, a flexible daily schedule, one-on-one patient care, and support from a collaborative team. GrandCare specializes in orthopedic rehabilitation, home health physical therapy, and neuro rehabilitation throughout Southern California.
Requirements
- Current California Physical Therapy License.
- At least 2 years of physical therapy experience.
- CPR certification.
- Valid driver's license and an insured vehicle.
Responsibilities
- Perform home health physical therapy evaluations and develop care plans.
- Provide therapeutic treatment and instruct patients and caregivers.
- Monitor patient progress and report changes to physicians.
- Participate in discharge planning and interdisciplinary case conferences.
- Supervise Physical Therapy Assistants according to state regulations.
