About The Position

Adoration Home Health is seeking a dedicated Physical Therapist to provide patient-centered home health care in Concord, North Carolina, covering Cabarrus and Mecklenburg counties. The role provides one-to-one care in patients’ homes and offers flexible scheduling options, independent work with team support, and opportunities for advancement. The schedule is Monday through Friday, 8 a.m. to 5 p.m.

The Physical Therapist evaluates and treats patients based on physician orders, establishes measurable goals and treatment plans, and coordinates with the treatment team, physicians, patients, and families. The therapist also maintains timely documentation and supervises Physical Therapist Assistants and Physical Therapy Aides in accordance with the state practice act.

Requirements

• Graduate of a Physical Therapist education program approved by CAPTE.
• Current Physical Therapist licensure in the state.
• American Heart Association CPR certification; certification may be offered and completed upon hire.
• Two years of experience in adult physical therapy.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills.
• Physical therapy skills as specified by the competency checklist.

Responsibilities

• Evaluate and treat patients based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and function.
• Maintain communication among the treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services to ensure proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
