About The Position
The Physical Therapist I evaluates, treats, and plans discharge for patients referred for physical therapy services. This healthcare provider helps patients achieve their highest level of physical, cognitive, linguistic, and swallowing function and works with other healthcare professionals to coordinate treatment plans and care. The role upholds Jefferson Health’s mission, vision, and values and is based at the Jefferson Torresdale outpatient location.
Requirements
- Bachelor’s, master’s, or doctoral degree from an accredited Physical Therapy program.
- 0–2 years of clinical practice required.
- Pennsylvania Licensed Physical Therapist credential.
- American Heart Association Basic Life Support certification.
Responsibilities
- Provide therapy evaluations, treatment planning, treatment interventions, and discharge planning for a patient caseload in appropriate settings.
- Participate in student education, including part-time or full-time student placements.
- Participate in education and teaching appropriate to a novice clinician.
- Plan, organize, and deliver treatment programs to support patients’ recovery and rehabilitation.
- Use tools and techniques to provide safe therapeutic interventions and training for patients and families.
- Record and document patient health progress and therapeutic interventions, including daily treatments, progress notes, evaluations, and discharge summaries.
- Coordinate treatment plans and care with other healthcare professionals.
- Interact with coworkers, visitors, and other staff in accordance with Jefferson’s values.
- Perform other duties as assigned.
